Do you need Client Intake Forms, Consent Forms and Treatment Forms (SOAP notes) for your massage business? Save time and purchase this bundle, which can be edited in the free program Canva.
I am a former Massage Therapist myself, so have designed these with Massage Therapists in mind! I have included the four major forms I used myself when I was in practice.
It has been designed with a rose gold and floral theme, but you can change that up to whatever colors, logos, and images you wish to use. You can also change any or all of the text on the form. Make it your own!
What Forms are Included?

2️⃣ Massage Consent Form (1 page)
3️⃣ Treatment Plan Form (1 page)
4️⃣ Ongoing Treatment Form (1 page)
All are 8.5x11" templates (letter size paper)
You receive:
❤️ A pdf manual with a link to your template on Canva, plus instructions on editing your templates to get the most out of them. There are also links on the last page to other tutorials on using Canva, if you need them.
HOW IT WORKS
After purchase, you will receive a PDF file as detailed above.
Access this file in the link that will be emailed to you.
ADDITIONAL NOTES:
- As this is a digital product, no physical items will be shipped to you. You will gain instant access.
- Print as many times as you like, and edit as many times as you like.
- You will need a free Canva account to edit these templates.
While you can edit almost everything, there are a few limitations, as follows:
** You cannot edit the size of the form (it is set at 8.5x11", typical letter size paper).
** You can only use the fonts that Canva provides.
- The print quality depends on your printer. Print at the highest print settings for best results.
- I cannot provide tech support for PDF software or printer settings. I do recommend trying Adobe Reader for iPad if you need something for that!
